Generic semaglutide is an FDA-approved glucagon-like peptide-1 (GLP-1) receptor agonist containing the same active ingredient and amino acid sequence as the branded versions Ozempic and Wegovy, both manufactured by Novo Nordisk. It works by mimicking a natural hormone your gut releases after eating, which triggers insulin release, slows gastric emptying to reduce food intake, and helps lower blood sugar levels.

Semaglutide is approved for two primary uses: blood sugar control in adults with type 2 diabetes, including cardiovascular risk reduction in patients with established cardiovascular disease, and chronic weight management in adults who meet specific body weight and BMI criteria when used alongside a reduced calorie diet and increased physical activity.

Dosing and dose escalation schedule

Semaglutide injection follows a gradual dose escalation schedule designed to reduce gastrointestinal side effects. Most patients start at 0.25 mg once weekly for four weeks, then increase to 0.5 mg. Depending on the indication and individual response, a prescriber may raise the dose up to 2 mg weekly for type 2 diabetes or 2.4 mg weekly for weight management.

If you have a missed dose, take it as soon as you remember, provided your next scheduled injection is more than two days away. If it is less than two days out, skip the missed dose and continue your regular schedule. Do not double up.

Common side effects

The following side effects are most frequently reported during dose escalation and tend to improve over time:

  • Diarrhea
  • Stomach pain and stomach cramps
  • Constipation
  • Vomiting
  • Abdominal pain
  • Decreased appetite

Serious side effects

  • Contact your licensed health care provider immediately if you experience any of the following:
  • Severe pain in your stomach area that does not go away, with or without vomiting (possible pancreatitis)
  • Changes in vision (possible diabetic retinopathy complications)
  • Signs of acute kidney injury or kidney failure: decreased urination, swelling in legs or feet
  • Signs of allergic reaction: swelling of face, lips, tongue, or throat; difficulty breathing; severe rash
  • Gallbladder problems: pain in your upper stomach, fever, yellowing of skin or eyes

Who should not take semaglutide

Semaglutide carries a boxed warning about thyroid C-cell tumors, including medullary thyroid carcinoma, a form of thyroid cancer observed in animal studies. Do not use semaglutide if you or anyone in your family has a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2.

Tell your prescriber about all other drugs you take. Combining semaglutide with insulin or other prescription medicines that lower blood sugar can increase the risk of hypoglycemia. Your doctor may need to adjust your insulin dose to account for changes in insulin production.

Semaglutide is not approved for use in children. Women who are pregnant or planning pregnancy should stop the medication at least two months before conceiving. It has not been studied in patients with a history of pancreatitis, and patients with chronic kidney disease or advanced liver fibrosis should discuss risks with their doctor.

Generic semaglutide vs. compounded semaglutide

These are not the same thing. Compounded versions may use different salt forms such as semaglutide sodium or semaglutide acetate, and compounded drugs do not go through the same regulatory review as FDA-approved generic drugs. A true generic version contains the same active ingredient in the same dosage forms and must pass a thorough review for bioequivalence before approval. When patent protections expire on a brand-name drug, generic manufacturers can produce the same molecule at lower cost because they are not required to repeat the original clinical trials.
